In 1999, the Salt Lake Valley Health Department convened the major Salt Lake County health care providers to discuss what could be done to address the needs of the growing number of uninsured in our community. The providers decided to form the Salt Lake Valley Health Care Coverage Coalition. Over the next two years, the Coalition identified several strategies that could be implemented to improve the system of care for the uninsured in the County. In 2001, the Coalition was awarded a Community Access Program grant from the federal Health Resources and Services Administration, and the Health Access Project was formed. Community Health Centers, Inc. administers the grant on behalf of the Coalition.

March 2000    Primary care safety net provider capacity assessment completed by Coalition
January 2001    Subcommittee chaired by Dr. Scott Leckman formed to develop plan for creating Volunteer Physician Network
May 2001    Coalition submits Community Access Program grant proposal to Health Resources and Services Administration to implement ideas for improving system of health care for the uninsured
September 2001    One million dollar Community Access Program grant awarded to Community Health Centers, Inc. on behalf of the Coalition
September 2001    Utah Medical Association passes resolution supporting creation of Volunteer Physician Network
November 2001    HAP kicks off physician recruitment at Salt Lake County Medical Society meeting
March 2002    HAP outstations case managers at three local hospitals
April 2002    HAP begins referring patients to HAP Volunteer Physician Network
June 2002    Over 250 physicians are participating in HAP Volunteer Physician Network; procedures in place for HAP patients to receive charity care at 6 hospitals
September 2002    HAP is awarded $700,000 continuation grant from Health Resources and Services Administration
February 2003    HAP receives 211 Hero Award for an Outstanding Agency
March 2003    All Salt Lake County hospitals are providing charity care to HAP patients being treated by Volunteer Physicians; HAP stations case managers at two additional hospitals
May 2008    HAP logs its 10,000th referral. Hap has served 4,600 clients and made over 11,000 appointments. In 2007 alone HAP volunteer providers donated nearly one million dollars in services, and Salt Lake valley hospitals donated 1.7 million dollars in services to HAP patients
