assistance to your patients:

Information and Referral (211 or 801-978-3333) Provides human
services information and referral for Salt Lake, Summit and Tooele
counties.
Children's Health
Insurance Program (CHIP) (1-888-222-2542) Provides health insurance
for children of working families who do not have insurance and do
not qualify for Medicaid.
Primary
Care Network (PCN) (801-538-6101) Provides health insurance
for adults 19-64 who have not had health care coverage for at least
6 months and who do not qualify for Medicaid.
Medicaid
(1-800-662-9651) Provides health coverage for children, pregnant
women, parents, refugees, immigrants (under certain emergency conditions),
and the aged, blind, and disabled. Income guidelines vary.
Salt Lake Valley
Health Department (801-468-2700) Provides various health related
services, such as immunizations, screening for Sexually Transmitted
Diseases, cancer screening, and Tuberculosis testing.
