External Evaluation
The external evaluation will be conducted by the Sphere Institute, with Dr. John Hornberger as the lead investigator. Sphere will provide input into the design of the evaluation, validate HAP data analysis and prepare the final report.
Primary goals:
  To assess the effect of HAP on reduction of client population’s mean number of ER visits
  To assess the effect of HAP on the client population’s mean rate of non-ER, hospital visits
  To assess the effect of HAP on the client population’s mean length of stay within the hospital
 
  Secondary goals:
  To assess the effect of HAP on client population’s hospital charges
  To assess the effect of HAP on client population’s quality of life
 
Internal Evaluation
HAP staff will conduct the internal evaluation, with outside input whenever possible on the design of survey instruments and methodology.
Goals:
  To assess client satisfaction with HAP services.
  To assess physician satisfaction with HAP services.
