TO QUALIFY FOR FOR OUR PROGRAM
Through our partners, Health Access Project provides free or at low cost medical care for low income and uninsured resident of of Salt Lake County. Our case Mangers are well trained and ready to support, help and answer your questions.
To qualify for program, you must be:

Salt Lake County Resident

Be Uninsured or insured

Have an income less than or equal to 150% of the Federal Poverty Guidelines ($19,750/per for an individual or $39,750/ year for a family of four).
